What is facilitated diffusion?

Using a cell as our example, there are times when larger molecules have to enter or leave a cell through the cell membrane. Since they are too large to p through like small water molecules, proteins in the cell membrane help these large molecules through the membrane, thus "facilitating" their movement through the membrane. As I remember it, this occurs with the concentration gradient. If it is against the concetration gradient, it becomes active transport.
